Without CLAUDE.md, Claude Code has no project context. It infers tech stack, naming conventions, and coding standards from whatever code it can see, and that inference is inconsistent across sessions.

One session uses camelCase. The next uses snake_case. One session writes tests. The next does not. Each inconsistency compounds across a project until manual correction costs more than the automation saved.

Plan mode causes Claude Code to produce a detailed plan before taking any action. For simple tasks, skipping it is reasonable. For complex tasks, skipping it is the most expensive shortcut in Claude Code workflows.

A wrong implementation across five files is not a Claude Code bug. It is the cost of not reviewing the approach before it was built. A wrong plan caught before execution costs one exchange. Wrong code built on a wrong plan costs a full correction cycle.

The review is not optional on any session, regardless of task size. A one-function change can introduce a SQL injection vulnerability. A configuration update can expose an API endpoint. Size does not correlate with risk.

  • Check logic correctness first: Does the implementation match the actual requirement, including edge cases that were not in the prompt?
  • Review for security patterns: Hardcoded secrets, injection vulnerabilities, and missing auth checks are common in AI-generated code and invisible to a casual read.

Mistake 5: Running Without Version Control

Every Claude Code session should start from a clean git state. No uncommitted changes, no untracked files that matter. A clean starting state gives you a precise before/after picture of everything Claude Code modified.

Without it, isolating what Claude Code changed is unreliable. Claude Code can modify files adjacent to the task, delete things that look like cleanup, or update configuration files alongside the primary change.

Mistake 6: Over-Using --dangerously-skip-permissions

--dangerously-skip-permissions disables the permission prompts where Claude Code asks for approval before deleting files, writing to specific paths, or making external network calls. Those prompts exist for a reason.

The flag was designed for sandboxed, automated environments: CI/CD pipelines and isolated Docker containers where no human is available to respond to prompts and the environment provides the safety boundary. That is not most development contexts.

  • Understand what it disables: Every file deletion, config overwrite, and external API call happens without confirmation when this flag is active.
  • The correct use boundary: An isolated container with no production access and no sensitive files is the appropriate context. A local development environment is not.
  • "Annoying prompts" is not a valid reason: If permission prompts interrupt the workflow frequently, configure CLAUDE.md to clarify what Claude Code is authorised to do. Do not disable the system.
